Am I Eligible?
Participation open to all residents of living care facilities in the state of Maryland. “Living care facilities” means an extended care facility, skilled nursing facility, rest home, convalescent home, convalescent hospital, home for the aged, or similar institution.

Can I Choose My Own Provider?
You have the freedom to choose your own dental and/or vision professional.

How Do I Make Appointments?
Appointments may be made through the living care facility. There is no prior authorization or review requirement with respect to accessing covered services.
